To be considered as a mentor in the Woman to Woman program at Northwestern Medicine, please fill out the application by clicking the button below.
If you are accepted as a Woman to Woman mentor, you will be required to complete a second application for NM Volunteer Services.

Mandatory qualifications to become a volunteer with Northwestern Medicine
You must be at least 16 years of age (18 years of age at Northwestern Memorial Hospital) to volunteer for Northwestern Medicine. In addition, volunteers must:
- Pass a criminal background check if you are age 18 or older
- Attend virtual hospital orientation for new volunteers
- Complete on-the-job training for assigned volunteer service (we will refer to this as "Woman to Woman Specific Training")
- Provide immunization records or undergo immunization screenings for:
- Measles, mumps, rubella (MMR)
- Fulfill current COVID-19 vaccine requirement and provide supporting documentation
- Chickenpox (varicella)
- Tetanus (Tdap)
- Others as needed
- Undergo a TB test
- Receive a flu shot if volunteering during flu season
- Read and comply with Northwestern Medicine policies, such as code of conduct, privacy laws and dress code
We request that volunteer mentors be at least 6 months post-treatment from Northwestern Medicine. However, we will consider mentors on a case-by-case basis if you do not meet the 6 months post-treatment request.
